    Message In A Bottle

    Message in a bottle is a joint scheme between Western Cheshire PCT and Lions clubs.
    A special bottle holds vital information for 999 emergency services and is stored in fridge.
    Every kit comes with three stickers to put on fridge door (green) and by front and back door (white).

      What information is included in the bottle?
    • Passport photo
    • Personal information - surname, first name, height, weight, date of birth, gender, hair colour and eye colour
    • Name, address and telephone number of the householder's next of kin or emergency contact
    • The name, or names, of all who live there
    • The name of the person's GP
    • Details about repeat medication
    • Basic information about any major medical conditions or allergies
    • Details about any pets
    • Details of those dependant on the householder - a child or older person
